We are in-network providers for most all PPO and EPO insurance plans, including, but not limited to:  Aetna, Blue Cross, Blue Shield, Cigna, Healtnet, and United Healthcare. 

We are preferred providers for Medicare, Tricare and Triwest.   We accept Workers Compensation and also provide cash rates for those patients not going through insurance.

We want your first visit at PacificPro to be a great experience and as smooth as possible. Please download the patient registration paperwork below, complete the forms and bring them in on your first day.  If you are unable to download the paperwork, please arrive 15 minutes early to your scheduled initial appointment to allow time to fill the forms out in the office.

Please 
arrive to our office 10-15 minutes before your first scheduled appointment. Be sure to bring your paperwork, a photo ID, your insurance card and the prescription for physical therapy if you have one. Please wear comfortable attire so your therapist can access your injured area. If needed, there is a bathroom to change your clothes. If the patient is a minor, a parent or guardian must accompany them on their first visit. We are looking forward to working with you!

Do I need a prescription/referral from a doctor to come in?
As of January 1, 2014, physical therapists in California can treat patients directly without a physicians prescription for up to 12 visits over 45 calendar days. Some insurance programs have been slow to adopt this change however. To be sure, contact your insurance provider or call one of our offices.


What insurance plans do you accept?
We accept most all PPO plans, Medicare, Triwest, Tricare and Workers Compensation. We also accept cash payments.  We do not accept any HMO insurances.

 
What do I need to bring to my initial visit?
Please bring your insurance card, identification, referral/prescription if available and your completed paperwork which can be found above on our website or e-mailed/faxed to you when scheduling your evaluation. 
 

What should I wear to physical therapy?
We recommend that patients wear loose fitting or athletic-type clothing that allows skin access to the body part being treated. Tennis or walking shoes are preferred.

 
How long will my appointment last?
Most appointments take anywhere from 60-90 minutes. If you are limited on time, please inform your therapist when you arrive and we can modify your treatment/exercise program to accommodate your needs.


Will I work with my therapist the entire time?
All manual therapy will be performed by your physical therapist. We also have physical therapy aides who are extensively trained to assist with patient's exercise programs. Those programs are created by your physical therapist specific to your needs/goals.


Is there a cancellation policy?
Yes, there is a 24-hour cancellation/no-show policy with a $25 charge.
